Over the past 5 years, greater scrutiny of the hospice program by the OIG has resulted in multiple investigative audits of hospices nationwide, identifying improper payments, ineligible admissions and recertifications, and very concerning trends of poor quality of care practices and noncompliance with the CoPs.

    Lista Clark began her nursing career after graduating BSN, RN from Vanderbilt University in her home town of Nashville TN. She has over 20 years’ experience in the Home Health and Hospice industry, beginning as a Primary Case Manager and moving into managerial and quality improvement positions over the years. Lista developed a passion and an expertise in documentation accuracy for Medicare and Medicaid regulatory requirements, and since 2011 she has been providing clinical consulting services to multiple home health and hospice agencies across the nation. Lista has significant experience in all types of MAC/RAC/TPE/UPIC Audits, Responses and Appeals, as well as Clinical Record Reviews, Survey Preparedness, CoP Compliance, QAPI, HIS, OASIS, and ICD-10 Compliance. CMS Contractor Post-Payment Medical Review continues to ramp significantly in many areas of the country with profound overpayment determinations being made.     Senior Healthcare Consultant

    Mr. Forrest began his career in 1979 as a Financial Auditor for a government contractor overseeing the Medicare Program. He entered the home health industry in 1992 as CFO for a home care operation in Nashville, TN.

    In 1994 he founded Forrest Consulting Services and has been a consultant to the industry since. In 2012 he founded Regulatory Compliance, LLC with the mission to partner with home care and hospice providers to help them navigate the ever-changing regulatory environment.

    Randy is a strong provider, advocate, and national speaker; training industry partners in compliant operational practices, regulatory requirements, changes, and reimbursement retention.

    As Director with Regulatory Compliance, he provides Interim Executive Management Services and works with a team of experienced home health and hospice clinical professionals to provide operational and clinical compliance oversight, appeals for claim denials, clinical due diligence reviews, operational reviews and cost reporting services. His rounded experience working on “both sides of the fence”, so to speak, is a great advantage for clients. Now as Co-Founder of S.M.A.R.T.
